Blaming is a lot like running in place, it takes up a lot of energy, but you really aren't getting anywhere. In the end, no one wins a game of blame.

Unfortunately, in systems as complex as health care, mistakes and missteps are going to happen. How we detect, report, prevent, and respond to errors is dependent on the culture of the healthcare organization. When something does goes wrong, the culture plays a role in whether the response is healthy (leads to learning & improvement!), or blaming and punishing.
