Unfortunately, in systems as complex as health care, mistakes and missteps are going to happen. How we detect, report, prevent, and respond to errors is dependent on the culture of the healthcare organization. When something does goes wrong, the culture plays a role in whether the response is healthy (leads to learning & improvement!), or blaming and punishing.
